A double chin is not always fat. It can be fat, loose skin, or the shape of your jaw. The right treatment depends on which one you have, and choosing the wrong one is the usual reason people are disappointed.
First, work out what is causing it
Pinch the skin under your chin. If you can hold a thick roll, that is fat. If the skin feels thin and the area sags when you lower your head, skin laxity is more likely. A small or set-back chin can make an ordinary amount of fat look bigger. Many people have a mix.

Chin liposuction
Liposuction removes fat in one procedure through a very small incision under the chin. It gives a more definite change than injections, and it suits larger amounts of fat. Fat dissolving injections
Injections suit a small pocket. Several tiny injections place a fat-dissolving compound under the chin, and the body clears the treated fat over the following weeks. Expect swelling that can last several days, and often more than one session. They are not a good fit for a large amount of fat or for loose skin. See our fat dissolving injections page.
HIFU
HIFU does not remove fat. It tightens. Focused ultrasound heats tissue under the skin so collagen builds, which can lift and firm mild laxity under the chin and along the jawline. Can you combine them?
Often, yes. A common plan is to treat the fat first, with liposuction or injections, and add HIFU later to tighten the skin. Your doctor will set the order and the gaps between treatments.
Who should not have chin liposuction
- People with very loose, thin skin under the chin, where removing fat could leave the skin hanging
- People whose double chin comes mainly from the shape of the jaw
- Anyone with a bleeding disorder or on blood-thinning medicine, unless a doctor says it is safe
- People who expect it to work as weight loss
Weight and your double chin
Losing weight can shrink fat under the chin, but it does not always do so, and it does not tighten skin. Many people reach a healthy weight and still have a double chin because of genetics and the way their neck is built. Treatments work best when your weight is stable.

How to decide
- Thick, clearly pinchable fat: chin liposuction is the most reliable.
- A small pocket of fat: injections can be enough.
- Loose skin with little fat: HIFU first.
- A receding or small chin: your doctor may also discuss chin shape.
