Where true glandular tissue is present beneath the nipple, a small periareolar incision (along the lower edge of the areola, 3–5 mm) allows direct excision of the glandular disc. This incision is placed within the natural pigmentation boundary of the areola — it heals with minimal visible scarring. Liposuction alone cannot remove firm glandular tissue; skipping this step in true gynecomastia cases leads to recurrence and an unsatisfactory contour.
